Bug/Issue Couple of bugs; quests can't complete, items/objects don't place.

Been sitting on one of these for a while, whereas one of them is new to 1.3. The first is the new one, wherein I get a quest that involves two NPCs and eventually it will turn out I get one where it's not completeable because the NPC is non-present. The log puts out the following when this happens:

The other problem that occurs pretty consistently is with dungeon generation. There are two drips and one keypad at the Outpost that never place correctly, and the log outputs that as a simple [Warn] message. However, this occurs very consistently on probably 75% of worlds visited. This now also includes space encounters as well. See below:

And so forth with that one. That was 3 out of 4 worlds and the only two static, forced dungeons visited yet since the update. Those results are consistent. I have wiped the install directory and reinstalled multiple times. Universe.chunks has been re-initialized since every update. Obsolete worlds that players do not occupy have been purged since 1.3. This is a small server that approximately 8 people play on regularly. We also have frequent crashes when people teleport back to their ships, but I currently don't have a log file for that.
