I suggest having ship malfunctions such as engine breaking or hull breaches. After a malfunction your ship would use emergency power to take you to the nearest planet to find resources to fix your ship ( eg. Iron or coal) these would occur rarely about 100 trip ratio

Sounds a bit annoying, honestly. The player doesn't really get anything out of it. Having it as a rare event is good, but I don't think it needs to be in the game at all.
Maybe it could be some kind of optional quest where your ship malfunctions and takes you to a place where you have to go through some sort of dungeon to fix it. An abandoned space station could work as a dungeon for this.
I agree with that view. It would just get irritating for that to happen all the time. If, maybe after 20 warps or so, this quest may occur, it could be kinda fun!
I totaly agree with your idea, it would add that awesome touch of unpredictable in the game but maybe as an option it's don't seem to please many people
