Bug/Issue Quest Giver Takes Wrong Item

• What happened? -- Twice I have had quest givers take the wrong item out of my inventory. In one case a quest giver took a parcel that was for a different quest giver, so I can't turn in the other quest and I still have a parcel for the first quest giver but no quest from him. The items were definitely next to each other in my inventory. The second time it took a legendary weapon out of my inventory. I am not sure if the quest item was next to the item it took.
• What were you doing in the game at the time? -- Turning in quests.
• Whether you were able to trigger the bug again by repeating what you did. -- It happened twice, identifiably.
• Your operating system name and version. -- Windows 10
• Are you using any mods? If so, which ones? -- No mods.
• Whether you're in full screen or windowed mode. -- Full Screen

As an additional note, I unbound the drop item button, so I know I did not throw my weapon on the ground by accident.
