Soldier's Syringe Unlock?

Quick question: Which achievement unlocks the Soldier's Syringe? It's one of my favorite items from the beta, and possibly one of the best tier one items in the game.

 

commando 'dodge 7 lethal attack'

So I got an issue, is there a reason it should not unlock? I have dodged more than 7 and it has not unlocked?

 

lower your health to below 20% and go underneath the golem boss. when he's about to stomp quickly dodge. thats how i unlock it

does it have to be consecutive?

 

I don't believe so, it'll likely even work if you dodge 7 separate lethal attacks from different targets in a single roll.
